Community Service Coordinator

Job Summary

The Community Support Coordinator – Health Access Program serves as a non-medical client health and wellness advisor, facilitator of material and service needs, and advocate and liaison between the client/family and community partners to the residents of Central Texas. CSCs are responsible for maintaining knowledge of program specifics such as application processes for Medicaid, SNAP, MAP, and other health services programs; self-starting searching for health and other service-related information that clients may be searching for inside and outside of AAUL and giving non-medical advice to clients through informed searches and using and demonstrating health literacy skills such as asking medical professionals. They also contribute to overall COVID-19 vaccine education and advocacy pertaining to the Urban League COVID-19 Vaccine initiatives.

Essential Duties and responsibilities

  • Responsible for: intake/screening/referral, assessment/reassessment (Virtual/In Person), development of support plans, on-going case management, monitoring of the client’s health and welfare, documentation of contacts and case management activities in the Department, resource development, and case closure.
  • Contact the clients at least once within each quarterly period, or more frequently if warranted by the client’s condition /or as determined by the program guidelines in which the clients are enrolled.
  • Primarily responsible for coordination of services to assist clients in increasing stability factors in their lives and accessing community resources; including but not limited to Austin community-based service agencies or 311 online applications or calls.
  • Maintains case files, records, and prepares monthly reports and time sheets.
  • Participates in supervision with Health and Wellness Director.
  • Works in conjunction with the AAUL staff and other AAUL programs.
  • Adheres to AAUL policies and procedures; supports an environment based in respect, the utilization of ethical communication, and adherence to the organizations nonnegotiable.
  • Attend Department provided training for AAUL as well as agency provided in-service and staff development training.
  • May take on special assignments and projects on an as needed basis.
  • Assist with system assessment and development as needed.  
  • Conveys the purpose and services of a program to the user population and the benefit to the community.
  • Establishes trusting relationships with clients and their families while providing general support and encouragement.
  • Conducts eligibility determination, enrollment, and follow-up with uninsured clients.
  • Provides referrals for services to community agencies as appropriate.
  • Helps clients connect with transportation resources and provides appointment reminders in special circumstances.
  • Works cooperatively with other clinical personnel assigned to the same client.
  • Provides consistent communication to evaluate client and family status, ensuring that provided information and reports clearly describe progress.
  • Ensures that all program applications submitted are complete with supporting documentation.
  • Maintains a minimal error rate in accordance with established guidelines.
  • Solid interpersonal skills with a high level of professionalism; in-person, phone, and email
  • Have a valid, State of Texas Driver’s License and Must have reliable transportation
  • Other duties as assigned by the Health and Wellness Director or President and CEO.

Knowledge, Skills and Abilities

  • 1 plus year of experience in health care experience or community involvement.
  • 1 plus year of experience creating and maintaining databases and reports for state grant funding.
  • Demonstrated interpersonal skills
  • Exhibit strong problem-solving abilities

Required Education

  • High School Diploma, GED, or equivalent Higher Education degree.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is asked to communicate with Health and Wellness Manager and AAUL President. The employee frequently is required to stand, walk, and lift or move boxes of supplies 20-50lbs to support community initiatives.

Work Environment

This job operates primarily in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Additionally, this position may operate in public parks and spaces, event centers, partner sites, etc. for various secondary events and initiatives.
